Your first office call will be a place to introduce each other, gather all necessary information, and determine a plan of action, including your desired goals and expectations.

- Most of this will be done through the initial patient intake forms.
- Bring in any past medical records that you may have and recent blood work within the past year. You will be asked to sign a release of records on your first visit in order to access these.
- All current and recently discontinued medications names, dosages, and frequency will be requested, including any supplements. The best and easiest way to ensure proper information exchange is to bring in all of your medications and supplements to your first visit.
