For all new patients, we ask that you fill out an intake form prior to your first visit.
Patients will not be seen until consent forms have been completely filled out.

The First Visit
Your first office visit will be an opportunity for us to introduce ourselves, gather essential information, and develop a plan of action that aligns with your goals and expectations.
Completing the intake form, which is quite detailed, can also be very helpful for identifying potential problem areas that we can address together.
What to Bring and Prepare:
General Assessment and Treatment Planning:
Labs are generally drawn during your first naturopathic visit, along with a general assessment. Treatment plans vary but often include meeting 2-4 times during the initial stages over 1-2 months. These early visits will focus on patient education, as well as creating and assessing the treatment plan. Afterward, treatment plans are implemented with a given duration and expected outcome, with ongoing telehealth visits and optional in-person visits for maintenance and changes along the way, normally every 2-12 weeks depending on patient needs. Beyond that, most patients are on an annual cycle of being seen.

What to Bring and Prepare:
- Initial Patient Intake Forms: Much of the necessary information will be collected through these forms, so please complete them thoroughly.
- Medical Records and Lab Results: Bring any past medical records and recent lab work from the past year. During your first visit, you will also be asked to sign a release of records so we can obtain additional information if needed.
- Medications and Supplements: Please bring all your current and recently discontinued medications, including supplements, with you to the first visit. This is the easiest way to ensure accurate information regarding names, dosages, and frequency.
